When you submit a “Book a Visit,” promotion, or contact form, you agree that:

submission does not guarantee availability, scheduling, or acceptance of an appointment,
we may contact you about your request and/or to provide service-related information.
6) No medical advice on the Site
Content on this Site is for general informational purposes only and is not medical advice, diagnosis, or treatment. Do not ignore professional medical advice because of something you read on the Site. In an emergency, call 911 or seek immediate care.
7) Communications and messaging terms (SMS/Text)
